How to Request Certificates/Academic Transcripts

We issue several kinds of certificates or academic transcripts both in Japanese and English for current and former students of Graduate School of Environmental Science, Hokkaido University. You can request certificates at a counter or by postal mail. Please read the directions below carefully.

Certificates are issued or posted two business days (excluding weekends and holidays) after we receive a request form. Certificates cannot be issued on the same day. Make sure to request certificates well in advance. 

【For Current Students】

  • How to Request
    1. Automatic Certification Machine (ACM) (Operating Hours: 9:00-17:00 on weekdays)
    2. At office (Office Hour: 8:30-17:00 on weekdays)
    3. By postal mail (See below◆)
  • Types of Certificates Available
    • Certificate of Enrollment (*)
    • Academic Transcript (*)
    • Certificate of Expected Completion (**)
    • Health Certificate (*)
    • Student Discount Certificate for Japan Railways (Up to 10 sheets each year) (*)
    • Certificates of Academic Records for Teacher’s course
      *Can be issued by ACM
      **Supervisor’s signature or stamp is needed on a request form (For M1, D1 and D2 Students ONLY)

【For Alumni】

  • How to Request
    1. At office (Office Hour: 8:30-17:00 on weekdays)
      • Please fill out a request form at a counter.
      • When the requester wishes to receive certificate(s) by postal mail, you must bring a stamped, self-addressed envelope.
      • When the requester receives certificate(s) at a counter, please show your valid ID such as a residence card, driver’s license or passport.

◆Request by postal mail

Please send items A through C to our office at the following address.

  • A: Request form
    1. Full name
    2. Certificates will be issued with the name used during one’s enrollment.
    3. If your family/given name has been changed and you want it updated, please provide Certificate of Individual Records (Koseki-Syohon) or Certificate of the Family Register (Koseki-Tohon)
    4. Date of birth
    5. Student ID number (if not known, the date of completion is enough)
    6. Divison
    7. Phone number
    8. E-mail Address
    9. Year and Month of Completion
    10. Purpose of Use
    11. The required types of certificates, number of sheets, and language (Japanese or English)
    12. Any additional information or requests
  • B: To prove the identity of the requester, include a photocopy (*) of the requester’s valid ID card such as a residence card, driver’s license or passport. *Copies will be properly discarded of after the identity of the requester is confirmed.
  • C: A stamped, self-addressed envelope

Envelope sizes

  • Up to 3 copies : Nagagata 3-gou envelope(120mm ×235mm ) accommodates folded A4 size.
  • 4 copies and more : Kakugata 2-gou envelope (240mm×332m) accommodates A4 size.

Postage

  • (standard-size envelope) 1 to 2 copies: 84 yen
  • (standard-size envelope)3 copies: 94 yen
  • (nonstandard‐size envelope)1 to 4 copies: 120 yen

If you request express mail, please add 260 yen stamps and write ‘Express mail’ in red on a return envelope. Please contact us if you would like to know the postage in advance.

Note

  • Certificates are issued or posted two business days (excluding weekends and holidays) after we receive a request form/return envelope. Certificates cannot be issued on the same day. Make sure to request certificates well in advance, considering the conditions of postal service.
  • There is no charge for issuing certificates. We only ask you for postage.
  • We do not mail overseas.
  • In case you cannot print out a request form, you can request by e-mail. (If the requester wishes to receive certificate(s) by postal mail, you must send us a stamped, self-addressed envelope.)
  • Please write the required information from 1 to 10 and attach a copy of your valid ID (*) in PDF, PNG or JPEG format.

    • Following items can be accepted as ID: Residence card, Passport, Health insurance card, Driver’s license etc.
    • We recommend you set a password to the file.
